Case Manager Full-time duties to include linking, coordinating, service brokerage, mobilization and advocating for a specific caseload of consumers in a variety of settings. Primary focus is meeting with individuals within their natural environments, coordinating community support systems and services, crisis intervention, education and assisting in removing barriers to improving functions.
